A front-desk form for trial and membership enquiries.
Gym Membership Enquiry Form: Fields, Questions and Follow-Up
By LeadFormHub Editorial Team ·
Quick Answer
What should a gym membership enquiry form ask?
Ask for a name, a mobile number, the goal, and when they want to start. Preferred time and membership length can wait until the call. There is no form template to install. You add each field by type and reorder it with the arrows.
What the front desk needs on the first call
A gym enquiry is a callback about a start date, not a long application. The person on the desk needs a name, a working mobile number, the training goal, and whether the caller wants to begin this week. Preferred time and the length of membership are useful later. They do not decide whether you pick up the phone.
People send this form from a story, a walk-in QR code, or a page about a trial. They are often on a phone. A medical history or a price list belongs after they agree to visit. Put the reason for the phone number in the label, because placeholder text is not saved as help.
Questions that sort a membership enquiry
Two closed questions are enough before the call. Goal tells the coach what to talk about. Start time tells the desk who to call first. Offer a way out on both, such as other or just exploring, so a real person is not forced into a false choice. Adjust the goal names to the programmes you actually run.
Goal options for a general gym are weight loss, muscle gain, general fitness, rehab or other. Replace that list if you do not coach one of them. Start options are this week, this month, and just exploring. This week is called first. Just exploring gets a follow-up date instead of an immediate tour.
- Goal. Weight loss, muscle gain, general fitness, rehab or other. The one dropdown. The coach opens with this.
- When to start. This week, this month, just exploring. The one radio group. This week is called first.
- Preferred time. Morning, evening, flexible. Ask on the call and write it in the note.
- Membership interest. Monthly, quarterly, annual, not sure yet. Ask on the call. Adjust the labels to your market.
How to place these fields when each type can be used once
The builder adds a field when you tap its type, and each type can be added once. Reorder with the arrows. Fields are not dragged into place, and there is no template to install. Name uses text. Mobile uses phone. Goal uses the one dropdown. When to start uses the one radio group.
That leaves no second dropdown for preferred time, membership length, and how they heard about you. Put those three in the conversation and store them in the note. UTM parameters, the referrer, and the landing page are already stored on every plan, including Free.
| Field | Type | Required | Why |
|---|---|---|---|
| Name | Text | Yes | The desk needs to know who asked. |
| Mobile | Phone | Yes | The callback number. Say that in the label. |
| Goal | Dropdown | Yes | Weight loss, muscle gain, general fitness, rehab or other. Adjust to your programmes. |
| When to start | Radio | Yes | This week, this month, or just exploring. |
| Preferred time | Ask on the call | No | Morning, evening, or flexible. A second dropdown is not available. |
| Membership interest | Ask on the call | No | Monthly, quarterly, annual, or not sure yet. Adjust the names to your passes. |
| How you heard about us | Stored on the lead | No | UTM, referrer, and landing page are saved on every plan. The dropdown is already used for goal. |
What the desk does when the enquiry arrives
Open the lead, read the goal and the start answer, and set a status, a note, and a follow-up date. Those three are on every plan. Call this week before just exploring. On Pro and Business an email alert can tell the account, and the Kanban board shows the pipeline. The product does not score the enquiry.
A practical split is simple. This week is called the same day. This month gets a follow-up date on the lead. Just exploring gets a later date. The free plan is 3 forms and 50 leads a month. Pro and Business remove that lead cap. Assignment needs a plan with team members.
A reply the member can receive by email
An automatic email is an automation rule on Pro and Business. It sends only when the lead has an email address. This form is phone-first, so add the email field or the rule does nothing. The message below is sample text. It uses the name placeholder the rule already supports.
Use the trigger for a new lead and the action that emails the lead. The subject and body are yours. {{name}} is replaced from the name field. Do not promise a transformation, a price, or a time you will not keep. The sample only confirms that the enquiry arrived and says what happens next.
Hi {{name}},
Thanks for telling us you want to start. A coach will use the goal you chose when they reply. If you said this week, expect a call on the mobile number you entered. If you are just exploring, reply with the month you have in mind and we will write back then.
This note confirms the form arrived. It is not a price or a training plan.Checking the mobile number
Turn on SMS OTP when a wrong number would waste a call. The code goes to India mobile numbers, not to every country. Email OTP works worldwide and needs an email field. Both are off the free plan and share one monthly count. Use the code on the trial form, not on a newsletter.
SMS OTP supports India mobile numbers; email OTP works worldwide. OTP is not on the free plan. Pro includes 100 verifications a month and Business includes 1000. Email and SMS codes share that quota.
Tell people why the code is coming, in the phone label. reCAPTCHA v3 can sit on the form as well. It reduces scripted junk. It does not prove the phone is real.
Publishing the membership form
Create a form and tap text, phone, dropdown, and radio. Set the options above. Use the arrows so name and mobile come first, then goal, then the start question. Share the link or embed the iframe. There is no template to duplicate. Submit one test lead before the link goes on an ad.
Test the form on a phone. The button should name the result, such as Book a trial call. Then check that the source stored on the test lead matches the link you shared.

Common questions
These answers cover the field limits, the missing template, and how verification and the reply behave on this form. The options above are a starting list. Adjust the names and the ranges to your own local market before you publish.
No. Add the name, phone, dropdown, and radio yourself, then reorder them with the arrows. Nothing on this page is a one-click template.
The builder allows one dropdown and one radio group. Goal and start date change the first call, so they take those two fields. Ask the other two when you speak.
